Self Referral to Physiotherapy & Podiatry Services

Self-referral Physiotherapy

This is a method of directly accessing outpatient Physiotherapy which is available for adults over 16 who are suffering from low back pain, neck pain, or who have recent injuries such as strains and sprains, or joint and muscular pain. For all other conditions you should consult your GP.

This referral option is not available to people under 16 years, if you have a neurological, paediatric, respiratory, obstetric or gynaecological problem, or if you are under the care of a consultant for this problem.

  • You can self-refer online to Bangor or Ards Community Hospital by clicking HERE.
  • You can pick up a paper version of the Self-Referral Form from the Trust Physiotherapy Departments, Health Centres or your GP Surgery

Self-Referral Podiatry

Podiatry Services are delivered by a team of Registered Podiatrists and are assisted by Podiatry Assistants at  Bangor and Ards Community Hospitals. The Podiatry Service provides a full range of acute and community podiatric interventions to people who have a medical need and related foot problems.

Referral to the Podiatry Service can be made by GP, other health care professional or by yourself using the departmental self-referral form available and returning it to Ards Community Hospital or Bangor Community Hospital

All referrals are triaged on receipt, as there are access criteria in place for NHS Podiatry treatment.
