Consent for the practice to communicate with patients via text and/or email

The practice is aware that there are times when it would be helpful to communicate with patients via SMS text or email. For example:

  • appointment reminders.
  • invitations to chronic disease clinics.
  • communicate requests to attend for blood tests.
  • advise re the need to speak to the doctor or nurse, or to book an appointment with the doctor or nurse.

Please note this system does not offer a reply facility to enable patients to respond to texts or emails directly.

If you give consent for us to communicate with you by mobile text messaging and/or email, please download and complete this form and return to reception.

To maintain the confidentiality of your information while using this system, it is ESSENTIAL that you let us know if you change your mobile number or email address.  If you decide you no longer wish to receive messages through this service, you can opt out at any time by contacting reception on 028 91515300.
