We extend a warm welcome to patients wishing to register with the Practice providing their address is within our practice boundary which is 4.5 miles from Bangor Health Centre.   Please view the Practice boundary map E280 4.5 mile MAP.You can change doctor without giving a reason or without the consent of your present doctor.

Patients joining us from another GP Practice within Northern Ireland or patients transferring from Great Britain need to complete a HS200 application form. The completed form needs to be brought in person to the surgery along with photographic ID and proof of address that you live within the practice area.

HS200 forms can be collected from reception or downloaded here.

Parents/Guardians of new babies are issued with an Infant registration form HS123 by the Registrar of births. Please bring this form to the surgery to register your baby.

Lesley and Carly from our reception team process registration paperwork between 10:00 am and 11:00 am on Tuesday, Thursday and Friday mornings. They will ensure all paperwork is in order and arrange a new patient medical appointment with Sr Sharon Smyth, Practice Nurse, to ensure we have the necessary clinical details to hand as it can take 6-8 weeks for patient notes to arrive from the previous practice.
